Nominations will be accepted Thursday, January 3, 2017, through 3 pm Eastern Time, Thursday, March 16, 2017. No exceptions will be made to this deadline.

Prior to submitting an application, you must review the following:

Review the entire application form before gathering information to note what information is required. All areas are required to be completed.

Once an application is started, you will be able to save and exit the form at any time during the application period to work on it before submission.

When you have filled out all required fields and uploaded your supporting documentation, and are ready to submit, please note that once you click on “SAVE AND FINALIZE” to submit your application, you will not have access to your application to make any changes. You will receive an e-mail notification that your application has been received.

NOTE: In most cases, the nominees and/or sponsors must be ASHA members in good standing. To check the membership status of your nominees and/or sponsors, search the Membership Directory on ASHA.org; their name will appear if they are a member in good standing.
